Description

Endoscopic treatment is the first choice in GI bleeding from ulcer or vascular lesions. TTS needles for injection of saline, epinephrine, or tissue glue; Disposable endoscopic flexible hemoclip for mechanic hemostasis; coagulation probes for destruction of vascular lesions, and catheters for hemostatic sprays (Fig. 6a, b) have expanded the therapeutic armamentarium. Larger OTS devices are available as rubber band ligation for routine treatment of varices and OTS clip as rescue therapy for bleeding ulcers. The disposable hemoclip can rotate 360⁰, opening angle can be 90°-135⁰ and it can be used in MRI.

Hemoclip therapy is now a well-established procedure in GI endoscopy, with several randomized studies demonstrating the clinical efficacy of this mechanical device in the treatment of bleeding peptic ulcers.

We can anticipate that the new natural orifice transluminal endoscopic surgery (NOTES) method and submucosal dissection techniques – which require postinterventional therapy for smaller perforations will encourage the development of novel clipping instruments. Our scope will be widened by this new flexible interventional endoscopic access to previously undiscovered places. New equipment will be required by the NOTES project, especially for sealing the punctured organs that are utilized to access the peritoneal cavity. In this situation, clipping and suturing devices will be very crucial and could result in major improvements in hemostatic therapy with hemoclips, both in terms of endoscopist convenience and patient outcome.
